Accommodation
form only for booking hotel by fax/courier
(otherwise use
information on previous form above for online/telephone booking)
Kindly print and complete the selected form(s) legibly
(1 form per delegate) and submit by fax to the UDOP Conference
Secretariat at 1 (876) 977-5233. Only forms that include
valid/accurate payment information will be considered for
registration and/or hotel bookings. The deadlines indicated on
the forms are effective at midnight Eastern Standard Time (GMT
-0500) on the dates noted.
